How to Write a Test Plan: Steps, Structure & Tips

A test plan is a document that spells out what you’re testing, how you’ll test it, who’s responsible, and what “passing” looks like. Writing one well means your team avoids ambiguity, catches gaps early, and has a shared reference point throughout the project. Whether you’re following a formal standard like IEEE 829 or working in a leaner agile format, the core process is the same: define scope, set success criteria, identify risks, assign resources, and document it all clearly enough that someone new to the project could pick it up and understand the strategy.

Start With Scope and Objectives

Before you outline a single test case, pin down two things: what you’re testing and why. The “what” includes specific features, modules, integrations, or user flows that need coverage. The “why” ties each item back to a business or technical goal, such as confirming a payment gateway processes refunds correctly or verifying that a new API endpoint handles edge cases without crashing.

Equally important is stating what you are not testing. The IEEE 829 standard for test documentation explicitly calls for a “features not to be tested” section, and for good reason. If your team assumes someone else is covering performance testing while nobody actually is, you’ll find out at the worst possible time. Writing down exclusions forces that conversation early. A clear scope section prevents both duplicated effort and blind spots.

Define Entry and Exit Criteria

Entry criteria are the conditions that must be true before a testing phase can begin. Exit criteria are the conditions that must be met before you can call that phase done. Without these, testing either starts too early (wasting time on broken builds) or drags on indefinitely because nobody agreed on what “finished” means.

For the test planning phase itself, typical entry criteria include having project requirements documented, the project scope defined, and a test strategy in place. Exit criteria for planning would be an approved test plan, finalized resource allocation, and a completed plan document. For test execution, entry criteria usually require that test cases have been developed and approved and test data has been prepared. Exit criteria include all test cases executed, defects logged, and results recorded with metrics.

Make these criteria specific and measurable. “Adequate testing completed” is vague and will cause arguments. “95% of critical-path test cases pass with zero severity-1 defects open” gives everyone the same target. Get input from developers, product owners, and other stakeholders when setting these thresholds so the numbers reflect real project priorities, not arbitrary benchmarks.

Choose Your Testing Approach

The approach section describes how testing will actually happen. This is where you lay out which types of testing apply (functional, regression, integration, performance, security, accessibility) and what techniques you’ll use for each. It also covers the balance between manual and automated testing.

For automated testing, specify the tools, frameworks, and agents that will run the tests. For manual testing, identify the group or individuals responsible for creating and executing test cases. If you’re using a mix, explain where the boundary sits. A common pattern is to automate regression and smoke tests while keeping exploratory and usability testing manual.

This section should also define your code coverage requirements if applicable. For unit testing, you might require that all public functions of a component are tested and that unit tests meet a specific coverage threshold. State the standard plainly so developers know what’s expected before they submit code.

Identify Risks and Mitigation Steps

Every project carries risks that can derail testing. Your test plan should name them explicitly and describe what you’ll do about each one. Common risk areas fall into a few categories.

  • Code risks: Legacy code that nobody fully understands, inherited services, or a codebase spread across multiple languages. These increase the chance of unexpected failures and slow down test creation.
  • Process risks: Error-prone manual steps, inability to deploy to staging daily, or outdated runbooks and documentation. If your team can’t reliably get a build into a test environment, no amount of test planning will save you.
  • Testing risks: Insufficient coverage, regression tests that aren’t automated or kept current, a high rate of false positives, or failure messages that aren’t actionable. These erode confidence in results.
  • Incident risks: A backlog of unresolved incident tickets, services that require manual restarts after dependent services recover, or recurring failures in the same components.

For each risk, decide whether you’ll avoid it, reduce it, or accept it. Avoidance means eliminating the risk entirely, like scheduling regular maintenance windows to upgrade outdated dependencies before they cause test failures. Reduction means taking steps to lower the probability or impact, such as adding monitoring to a fragile service. Acceptance means acknowledging the risk exists and choosing to move forward, typically because the cost of mitigation outweighs the potential damage. Document all three categories so the decision is visible to the team.

Plan Your Test Environment

The environment section specifies the hardware, software, network configurations, databases, and third-party services needed to run your tests. This includes operating systems, browser versions, mobile devices, and any staging or sandbox environments that mirror production.

Be precise. “Test on major browsers” is not a plan. “Chrome 125+, Firefox 126+, Safari 17+, Edge 125+ on Windows 11 and macOS Sonoma” is a plan. If you need specific test data, seed databases, or mock services, document those requirements here so environment setup doesn’t become a bottleneck when execution begins.

Assign Roles, Resources, and Schedule

A test plan needs clear ownership. List every testing task and assign it to a specific person or team. Include staffing needs and any training required. If your team has never used a particular automation framework, that training time needs to appear in the schedule, not surface as a surprise two days before a deadline.

The schedule section maps testing activities to dates or sprint cycles. Communicate testing times to everyone involved, not just the QA team. Developers need to know when builds are expected. Product owners need to know when results will be available for review. A test plan that lives only inside the QA team’s head isn’t a plan at all.

For each milestone, tie back to your entry and exit criteria. If the entry criteria for test execution aren’t met by the scheduled start date, the plan should specify what happens: does the schedule slip, does the scope shrink, or does the team escalate?

Define How Results Are Stored and Reported

Testing generates a lot of data, and your plan should explain where it goes and who sees it. Describe the logging or database technology used to capture results, where the data will reside, and how it will be accessed. If your organization needs to maintain testing history for compliance, audits, or trend analysis, account for that storage requirement up front.

Reporting is just as important as storage. Your plan should include a detailed list of reports to be issued, their intended recipients, and the distribution method. A development lead might need a daily summary of pass/fail rates, while a project sponsor might only need a weekly status report with defect trends. Tailor the format and frequency to the audience.

Quantitative standards matter here. Define how success is evaluated for each report. If your nightly regression run shows 98% of tests passing, is that a green light or a red flag? The answer depends on what you established in your exit criteria, which is why these sections reinforce each other.

Keep the Plan Under Version Control

A test plan is a living document. Requirements change, scope shifts, and new risks emerge. Treat your plan with the same discipline you’d apply to code: put it under version control and use semantic versioning (for example, v1.0 for the initial approved plan, v1.1 for minor scope adjustments, v2.0 for a major overhaul after a release pivot). This ensures everyone works from the same version and changes are traceable.

Review and update your entry and exit criteria regularly as well. What made sense at the start of a project may not hold after three sprints of changing requirements. Schedule periodic reviews of the plan itself, not just the test results.

  • Approvals: Who signs off on the plan before execution begins.

You don’t need to follow this list rigidly. A small agile team might collapse several sections into a single page. A large enterprise project might expand each section into its own document. The goal is the same either way: make sure everyone involved in testing knows what’s being tested, how, by whom, and what “done” looks like.
